After completing the form you can email it to Rick at rick@lvpoa.org. To do this you must scan the form in and send it as an attachment. You may also mail it to us or bring it to the office. Mailing address: PO Box 4766 Lago Vista, TX 78645.
As a reminder the date will not be recorded as reserved until we recieve the completed form and the Facility Manager has had an opportunity to review your completed form with you to ensure that we can meet your expectations.
